What to include with your application

Attach the documents that apply to you.

Proof of age (elderly)

  • Driver's license
  • State ID card
  • Birth certificate
  • Passport

Proof of disability

  • SSA Disability Award Letter
  • SSI Disability Determination
  • Physician's certification
  • State or federal disability benefit documentation

Proof of disabled veteran status

  • VA Award Letter showing 100% permanent and total disability
  • VA Benefits Summary Letter (VBA)

Unmarried surviving spouses also need:

  • The veteran's disability documentation
  • Death certificate
  • Proof of marriage
  • Proof you have not remarried

Proof of income (all sources)

Report all income using your IRS Federal Tax Return (Form 1040). If you did not file a return, provide your most recent Social Security SSA-1099 statement.

We also accept:

  • W-2 wage forms
  • 1099s (Social Security, pensions, retirement, interest, dividends)
  • Pension or retirement income statements
  • Interest, dividend, or investment income statements
  • Business or rental income records
  • Any other documents showing all sources of income
